Patient Feedback Privacy Notice

Loaro Surgical Clinic uses feedback submitted through this form to understand patient experience, recognise good service and improve clinic quality. The feedback form is separate from the appointment process and is not monitored as a request for medical advice or urgent care.

Information we collect

• Your overall experience rating
• Staff and communication ratings, comments or suggestions only if you choose to provide them
• Your name and email address only if you choose to provide them
• Your optional choice about whether the clinic may contact you regarding the feedback
• Basic technical and security information needed to protect the form from misuse

Anonymous and identifiable feedback

You may leave the name and email fields blank. Providing an email address does not automatically give the clinic permission to contact you. The clinic will use it to follow up on the feedback only if you also select the optional contact-permission checkbox. Anonymous feedback may not allow the clinic to investigate an individual matter or reply to you.

How we use it

Authorised clinic staff review feedback for service improvement, staff learning, quality assurance and appropriate follow-up. Where possible, reports and improvement discussions use combined or de-identified themes rather than identifying the person who submitted the feedback.

Please exclude confidential clinical details

Do not include medical records, identity numbers, photographs, a detailed medical history or information identifying another patient. To raise a clinical complaint or patient-safety concern that requires investigation, contact the clinic directly so it can be handled through the appropriate process.

Sharing and retention

Feedback is not sold or used for advertising. It may be accessed by authorised clinic personnel and service providers supporting the website or email system, subject to appropriate safeguards, or disclosed where required by law or necessary to protect a person. It is retained only for as long as reasonably necessary for quality improvement, follow-up, security and legal obligations.

Your privacy rights

Subject to applicable Botswana law, you may ask about identifiable personal information held about you and request access, correction or deletion where appropriate. Contact info@loarosurgicalclinic.com. The clinic may need to verify your identity before acting on a request.

Emergencies and medical advice

Do not use this form for an emergency, diagnosis or treatment advice. Go directly to the Riverside Hospital Emergency Room or contact the appropriate emergency service.
